Empath Home Health in Ocala, FL is seeking a compassionate full-time Home Health RN to provide in-home skilled nursing and patient education across Marion, Lake, and Sumter counties.

You will perform OASIS assessments, develop plans of care, and collaborate with the interdisciplinary team to support patients' independence and Full Life Care.

This role includes weekend/holiday on-call rotations and opportunities for CEU support, tuition reimbursement, and career growth.

Qualifications

• Graduate of an approved nursing program with an active Florida RN license.
• Prefers BSN or MSN degree.
• Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred.
• Strong assessment, critical thinking, and patient education skills.
• Willingness to participate in weekend and holiday on-call rotations.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled nursing services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.
