Job summary

Hospice of Marion County, part of Empath Health, seeks an RN (PRN) to visit patients after hours in homes and facilities around Marion County, providing clinical, educational, and emotional support. You will assess needs, coordinate with the Interdisciplinary Team, and educate families with compassionate care.

Requires Florida RN license and valid driver’s license; one year of acute or post-acute care is preferred. Join a mission-driven team delivering Full Life Care for All.

Qualifications

• Associate's degree (A.A.) or equivalent from two-year college or technical school.
• Minimum of one year related experience and/or training.
• Valid Florida RN license.
• Valid Florida Driver's License.
• Prefer one year experience in acute or post-acute care; experience with terminal patients is a plus.

Responsibilities

• Visit patients to provide clinical, physical, educational and emotional support after-hours in patient homes, long-term care facilities, and assisted living facilities in Marion County.
• Obtain patient history and perform age-specific physical, psychosocial and spiritual assessments; initiate referrals and coordinate with the IDT, physician and partnering facilities.
• Identify learning needs of patients and caregivers; use teaching strategies and follow-through.
• Documentation is compliant with organizational standards including productivity and timeliness.
• Support families after a patient's death in collaboration with the IDT.
• Direct and supervise care provided by LPNs and Home Health Aides; evaluate Home Health Aide services biweekly.
• Educate IDT members regarding nursing aspects of patient/family care; represent hospice care philosophy to the community.
• Administer, monitor and document medications as prescribed; provide patient/family teaching.

If viewing in HTML, please see* *in English or* *in Spanish.***Hospice of Marion,** a Part of **Empath Health**, is seeking an **RN (PRN)** to join ourteam visiting patients to provide clinical, physical, educational and emotional support after-hours in in the patient homes, long term care facilities and/or assisted living facilities in **Marion County**.* Demonstrates the ability to obtain patient history, perform an age-specific physical, psychosocial and spiritual assessment and initiates appropriate referral, intervention and collaboration between the patient, family, Interdisciplinary team (IDT), physician and partnering facility (as applicable) following policy and practice guidelines.* Identifies age-specific patient and caregiver learning needs, motivations, barriers, abilities and preferences and utilize teaching strategies/instructions and follow-through to match.* Documentation is compliant with all organizational standards including productivity, timeliness and adheres to relevant policies and practice guidelines.* Following a patient's death, assists with closure contact and support in collaboration with IDT.* Directs and supervises the care provided by the LPN and Home Health Aide following agency policy. Evaluates services provided by the Home Health Aide every two weeks with periodic on-site supervisory visits.* Educates and represents the community regarding hospice care philosophy and concept through a case by case consultation with persons in the community or educational inservice or seminar as assigned.* Provides education to IDT members regarding nursing aspects of patient/family care.* Demonstrates the ability to administer, monitor and document medications as prescribed by attending physician.
