Job summary

Empath Health, through Tidewell Hospice, seeks an RN Admissions to coordinate patient admissions across home, hospital, and long-term care settings in the Sarasota area.

You will educate patients and families about disease progression, care options, and safety, and perform initial assessments of needs related to terminal illness.

Qualifications

• Active Florida RN license required.
• Hospice experience preferred.
• Experience in acute-care settings with patient/family education.

Responsibilities

Provides hospice admissions throughout the community, including patients' homes, hospitals, assisted living facilities (ALFs), and long-term care (LTC) facilities.
