Job summary

Aspirus St. Luke's Hospital in Duluth, MN is looking for an RN- Acute Care Coordinator to join the CARE COORDINATION team. This part-time (0.9 FTE, 72 hours biweekly) position is scheduled Day shift, 8 hours, with occasional weekend requirements.

The RN will assess patient needs, coordinate referrals to resources, and develop comprehensive care plans. Minnesota RN licensure, an associates degree, and acute care experience are required; BSN is preferred.

Qualifications

• Associates Degree in Nursing required; BSN preferred.
• Minimum one year of acute care hospital experience.
• Current RN licensure in Minnesota.
• Strong interpersonal, discharge planning and prioritization skills.
• Ability to organize, coordinate and advise across disciplines.

Responsibilities

• Assess patient needs and coordinate referrals to resources to meet those needs.
• Develop the patient's plan of care and discharge planning.
• Collaborate with physicians, nursing and ancillary departments.
• Provide clinical guidance and support to nursing personnel.
