Qualifications

• Basic Life Support certification.

Responsibilities

• Assess psychosocial and resource needs to establish comprehensive care and discharge plans.
• Coordinate care progression and communicate patient progress to ensure timely medical necessity.

They collaborate with physicians and care teams to develop and implement efficient treatment and discharge plans, ensuring timely progression of care and resource utilization.

Key Responsibilities:

• Assessment: Conduct psychosocial and resource assessments, partnering with interdisciplinary teams and payers to establish comprehensive care and discharge plans.
• Care Progression: Facilitate communication about patient progress, identify and resolve delays, and ensure medical necessity for treatments.
• Disposition Planning: Manage discharge planning, develop individualized post-acute care strategies, and coordinate referrals to ensure smooth transitions.
• Documentation: Accurately record assessments, interactions, and care plans, tracking trends to improve outcomes.
• Precepting/Mentoring: Support onboarding, mentor new staff, and serve as a resource for interns and less experienced colleagues.
• Professional Development: Complete required training, participate in goal-setting, and contribute to organizational success.

Qualifications & Skills:
