Job summary

Aspirus Medford Hospital in Medford, WI is seeking an RN Ambulatory Care Coordinator. This full-time, 1.0 FTE hybrid role involves coordinating care for patients with chronic conditions across Aspirus Clinic System, collaborating with the health care team.

Requirements include a Wisconsin RN license, BLS, ACLS/NRP as applicable, BSN or progress toward degree within 4 years, and professional nursing membership. Strong leadership and patient care skills are essential.

Qualifications

• Knowledge of patient care standards and practices normally acquired through completion of a nursing program.
• Two years' experience in department/specialty; supervisory/leadership experience beneficial.
• Active Wisconsin RN nursing license.

Responsibilities

• Assess, coordinate, and integrate healthcare for patients with chronic conditions within the Aspirus Clinic System.
• Collaborate with physicians, staff, and other health professionals to provide services across the care continuum.
• Deliver population health management practices to patients and the community.
