The RN- Acute Care Coordinator is responsible for assessing patient needs and coordinating referrals to resources to meet those needs. The RN- Acute Care Coordinator uses nursing practice knowledge in the development of the patient's plan of care. As part of the multidisciplinary team, the RN- Acute Care Coordinator works closely with physicians, nursing and ancillary departments, and provides support and clinical expertise to nursing personnel regarding patient care coordination issues. The RN- Acute Care Coordinator attempts to resolve social, economic and environmental difficulties that interfere with the patients' ability to manage their health needs effectively. The RN- Acute Care Coordinator requires independent judgment, analytical, problem solving, and collaborative skills in order to organize and prioritize multiple tasks to provide patient centered care.
Knowledge and skills normally acquired through completion of a Associates Degree in Nursing. BSN preferred.
Minimum of one year acute care hospital experience. Experience in critical care, general medical and/or surgical nursing, and experience in educational presentations beneficial.
Previous acute care hospital experience preferred. Acute care hospital internship recommended
Possesses current RN licensure, in the state of business units assigned.
Annual competencies as required by Aspirus and/or various regulatory agencies based on entity and/or job position.
