Job summary

Kindred Hospital Louisville at Jewish Hospital seeks a Registered Nurse to provide planning and delivery of comprehensive patient care using the nursing process, from assessment to evaluation, with emphasis on discharge planning.

You will supervise care, document accurately, administer medications safely, and collaborate with physicians and interdisciplinary staff to support patient recovery in a 30-bed LTACH setting.

Qualifications

• Postsecondary nursing education is required.
• Associate or Bachelor’s degree in Nursing preferred.

Responsibilities

• Provide nursing care planning and delivery using the nursing process (Assessment, Planning, Intervention, Evaluation).
• Direct, supervise, and evaluate nursing care and ensure patient safety.
• Document admissions assessments, care plans, and updates in the medical record.
• Administer medications following the five rights and monitor patient responses.
• Participate in discharge planning and collaborate with interdisciplinary team.
• Assist physicians with examinations, treatments, and procedures as needed.
• Provide end-of-shift reports and ensure accurate handoffs.
• Ensure equipment safety and proper operation and participate in orientation of new staff.
