Job summary

Kindred Hospital Louisville at Jewish Hospital is seeking a skilled care coordinator to support the Interdisciplinary Care Transitions team. The role ensures seamless patient flow, optimal resource use, and continuity of care across the continuum in a 30-bed long-term acute care setting.

You will collaborate with physicians, nurses and social workers, assess patient needs, plan care, and participate in rounds to align discharge planning with payer requirements and regulatory standards.

Qualifications

• Must read, write and speak fluent English.
• RN licensure required or eligible for compact licensure.
• Experience: 2+ years in healthcare; case management or discharge planning preferred.
• Postsecondary nursing education required; bachelor’s degree preferred.

Responsibilities

• Coordinate clinical and psycho-social activities with the Interdisciplinary Team and Physicians.
• Monitor patient stays for effective care coordination and efficient care facilitation.
• Refer high risk patients for additional support.
• Participate in interdisciplinary rounds to review goals and plan of care.
