Qualifications

• Graduate of an accredited Registered Nursing (RN) program.
• Current and active RN license in the state of practice.
• Current BLS/CPR certification.
• SNF/LTC, rehab, or geriatric nursing experience preferred.
• Minimum of 2 years nursing experience preferred; new grads with strong clinical skills are encouraged to apply.

Responsibilities

• Provide direct nursing care to residents in accordance with individualized care plans.
• Perform resident assessments and monitor changes in condition.
• Administer medications and treatments safely and accurately.
• Complete nursing documentation and maintain accurate medical records.
• Develop, implement, and update resident care plans.
• Communicate effectively with physicians, families, therapists, CNAs, and interdisciplinary healthcare teams.
• Supervise and support nursing assistants to ensure quality resident care.
• Respond appropriately to resident needs, emergencies, and changes in health status.
• Follow all state, federal, and facility regulations, including infection prevention and safety standards.
• Maintain a safe, clean, and compassionate environment for residents.
