Job summary

Immediate Home Health and Hospice in the Philadelphia area seeks a compassionate Registered Nurse to provide skilled in-home care. You will deliver quality nursing services in patients’ homes with focus on safety and comfort.

Responsibilities include SOC visits, planning and coordinating care with the team, completing admissions, documenting OASIS, and educating patients and families on disease management and home safety. Flexible hours and strong support are offered.

Qualifications

• Must hold current, active Registered Nurse (RN) license.
• Valid driver's license and reliable transportation.
• At least one year of nursing experience; home health and OASIS experience preferred.
• Knowledge of OASIS, Medicare Conditions of Participation, and home health regulations.
• Strong assessment, critical thinking, and time management skills.
• Proficient with EMR systems; WellSky Kinnser experience is a plus.

Responsibilities

• Conduct daily Start of Care (SOC) visits and comprehensive patient assessments in the home.
• Develop, implement, and evaluate individualized plans of care with physicians and the care team.
• Complete the admission process, including informed consent and review of patient rights and policies.
• Accurately complete OASIS documentation and ensure CMS/regulatory compliance.
• Communicate findings, orders, and care coordination needs via Admission report.
• Provide skilled nursing care, including medication administration and wound care.
• Educate patients and caregivers on disease management, medications, and home safety.
• Respond to urgent patient needs and clinical situations promptly and professionally.

What You Will Do:

• Conduct daily Start of Care (SOC) visits and comprehensive patient assessments in the home

• Complete the admission process, including obtaining informed consent, reviewing patient rights, advance directives, emergency preparedness, and agency policies.
• Accurately complete OASIS documentation and ensure compliance with Medicare, Medicaid, state, and agency regulations
• Communicate assessment findings, admission orders, care coordination needs, visit frequencies, and identified risks to the Clinical Management team and physicians via Admission report.
• Provide skilled nursing care, including medication administration, wound care, treatments, and patient education
• Educate patients and caregivers on disease management, medications, home safety, and the home health plan of care
• Respond to urgent patient needs and clinical situations promptly and professionally
