Job summary

Immediate Home Health and Hospice in Philadelphia, PA is seeking a compassionate RN to conduct Start of Care visits and home assessments, developing individualized plans of care with physicians and the care team.

You will complete admissions, obtain informed consent, review rights and directives, perform OASIS documentation, ensure Medicare and state compliance, and deliver skilled nursing care and patient education.

Qualifications

• Current, active RN license and eligibility to practice in PA.
• Valid driver's license and reliable transportation.
• At least one year of nursing experience; home health and OASIS experience preferred.
• Knowledge of OASIS, Medicare Conditions of Participation, and home health regulatory requirements.
• Proficiency with EMR systems (WellSky/Kinnser is a plus).
• Strong assessment, critical thinking, and time management skills.

Responsibilities

• Conduct Start of Care (SOC) visits and comprehensive home assessments.
• Develop, implement, and evaluate individualized plans of care with physicians and the interdisciplinary team.
• Complete admissions, including informed consent, patient rights, advance directives, and agency policies.
• Accurately complete OASIS documentation and ensure Medicare/state compliance.
• Communicate assessment findings, orders, and care needs to Clinical Management and physicians.
• Provide skilled nursing care, including medications, wound care, and patient education.
• Educate patients and caregivers on disease management and home health plan of care.
• Respond promptly and professionally to urgent patient needs.

Responsibilities

• Conduct daily Start of Care (SOC) visits and comprehensive patient assessments in the home
• Develop, implement, and evaluate individualized plans of care in collaboration with physicians and the interdisciplinary care team
• Complete the admission process, including obtaining informed consent, reviewing patient rights, advance directives, emergency preparedness, and agency policies.
• Accurately complete OASIS documentation and ensure compliance with Medicare, Medicaid, state, and agency regulations
• Communicate assessment findings, admission orders, care coordination needs, visit frequencies, and identified risks to the Clinical Management team and physicians via Admission report.
• Provide skilled nursing care, including medication administration, wound care, treatments, and patient education
• Educate patients and caregivers on disease management, medications, home safety, and the home health plan of care
• Respond to urgent patient needs and clinical situations promptly and professionally

Qualifications

• Current, active Registered Nurse (RN) license
• Valid driver's license and reliable transportation
• At least one year of nursing experience; home health and OASIS experience preferred
• Knowledge of OASIS, Medicare Conditions of Participation, and home health regulatory requirements.
• Strong assessment, critical thinking, and time management skills
• Proficient with electronic medical records (EMR); WellSky Kinnser experience is a plus
