About This Role

As a Registered Nurse in Home Health, you will provide and direct nursing care to patients in their homes as prescribed by the physician, ensuring compliance with applicable laws and agency policies. This role is ideal for nurses who are passionate about delivering high-quality care and coordinating with healthcare professionals.

What You'll Do

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses.
• Provide required supervisory visits.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements.
• Ensure patients meet home health eligibility and medical necessity guidelines.
• Communicate relevant information timely and effectively with appropriate agency staff.
• Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
• Participate in on-call and weekend rotation as needed to meet patient needs.
• Adhere to and participate in the agency's utilization management model.

Requirements

• Education:
  • Current Occupational Therapy licensure in state of practice
• Experience:
  • Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
• Licenses & Certifications:
  • Current CPR certification or ability to obtain within 90 days of employment
