Job summary

Continuum Healthcare Inc. is seeking a Registered Nurse (RN) to join Serenity Spring Senior Living at Jasper Rehabilitation and Nursing Center in Jasper, IN. This full-time role focuses on supervising nursing staff and delivering high-quality sub-acute and rehabilitation care to residents.

The RN will collaborate with the Director of Nurses and ADON, monitor resident conditions, and ensure accurate MARs and nursing notes. A valid IN RN license and LTC experience are required.

Qualifications

• Active Indiana RN license required.
• Minimum 1 year of long-term care experience.
• Experience supervising nursing staff preferred.

Responsibilities

• Supervise nurses and aides to ensure quality resident care in a long-term care setting.
• Assess residents and document changes in care plans and MARs.
• Coordinate with DON/ADON and attending physicians regarding resident needs.
• Ensure accurate documentation of nursing notes and treatment records.
• Monitor supplies and equipment to ensure availability and functionality.
• Liaise between nursing staff and physicians, including after-hours calls when needed.

Responsibilities for Registered Nurse (RN):

• Work with the facility's Director of Nurses and Assistant and implement recommended changes as required
• Responsible to supervise nurses and aides for Long Term Care facility
• See residents and monitor resident care and report changes to the Director of Nursing (DON) or Assistant Director of Nursing (ADON)
• Assess newly admitted or readmitted residents and document in the 24 - hour report. Follow up on residents as needed
• Monitor nursing staff to ensure proper documentation on MARs, nurses notes, accident/ incident reports, physician's orders and treatment records
• Liaison between nursing staff and attending physicians. Assess the necessity of calls to the physicians. Assess the necessity of calls to the physicians during off business hours
• Checks to see that all supplies are in proper place and all equipment is functioning
