Job summary

Duke Health seeks a Clinical Nurse II – Rheumatology to join our telehealth-enabled care team in Durham, NC. The role focuses on nursing triage, coordinating care with rheumatology providers and families, and guiding patients through post-procedure instructions.

The position is based in Durham with a flexible hybrid schedule after onboarding, and involves collaboration with pulmonary services to optimize patient outcomes.

Qualifications

• Graduation from an accredited BSN or Associate's Degree in Nursing or Nursing Diploma program.
• RN licensure in North Carolina (current or compact).
• BLS certification.

Responsibilities

• Provide nursing triage, assessment and education to patients via telephone interactions and secure messaging.
• Manage patient needs including medication management, post procedure instructions and care coordination.
• Serve as liaison to patients, family members and referring providers with guidance from pulmonary providers.
• Collect and analyze patient data, make recommendations for nursing care plans, document actions and patient responses.
• Delegate tasks and direct activities of unlicensed care providers.
• Participate in professional development and performance improvement activities.
