Qualifications

• Graduate from an accredited Associate's Degree in Nursing or Nursing Diploma program.
• Minimum 2 years of nursing experience and 1 year of med-surg experience.
• Current NC RN licensure; BLS required.

Responsibilities

• Provide nursing triage, assessment and education to patients via telephone interactions and secure messaging.
• Manage patient needs including medication management, post procedure instructions and care coordination.
• Serve as a liaison to patients, family members and referring providers with guidance from pulmonary providers.
• Collect and analyze patient data, make recommendations for nursing care plans, evaluate nursing care provided, document and communicate actions taken.
