Qualifications

• Minimum degree in human services, social services, behavioral health, workforce development, or related field.
• Experience in case management, employment services, or community outreach preferred.
• Familiarity with recovery support and confidentiality practices preferred.

Responsibilities

• Serve as the primary point of contact for participants throughout the Return-to-Work journey.
• Conduct outreach to new participants and explain available program services.
• Coordinate intake, assessments, and work-readiness screenings.
• Develop individualized Return-to-Work plans based on goals and needs.
• Help participants navigate job searches, interviews, and onboarding.
• Coordinate conversations about work restrictions and accommodations with authorized parties.
• Connect participants with clinical, recovery, and community resources.
• Travel locally for participant meetings, employer engagement, and partner visits.
• Maintain participant documentation and participate in program reviews.
