Job summary

Ascension Recovery Services is seeking a Return-to-Work Liaison to work directly with individuals navigating substance use disorder, recovery, health-related challenges, and barriers to employment. This role emphasizes solving problems, coordinating resources, and helping participants move toward greater stability and independence through meaningful employment.

The position involves building individualized plans, assisting with job searches, and coordinating resources to support transitions back

Qualifications

• Experience in case management, employment services, community outreach, recovery support, workforce development, or related fields preferred.
• Experience working with individuals affected by substance use disorder or barriers to employment valued.
• Strong interpersonal and relationship-building skills.
• Excellent organization, documentation, and follow-through.
• Ability to travel locally for participant meetings and community visits.

Responsibilities

• Serve as the primary point of contact for participants throughout their Return-to-Work journey.
• Conduct outreach to new participants and explain available program services.
• Complete participant intake and coordinate assessments and work-readiness screenings.
• Identify employment barriers and develop individualized Return-to-Work plans.
• Help participants navigate job searches, applications, interviews, onboarding, and workplace expectations.
• Coordinate conversations regarding work restrictions and accommodations with authorized parties.
• Connect participants with clinical, recovery, rehabilitation, wellness, and community resources.
• Coordinate warm handoffs to outpatient care, MAT, and recovery supports.
• Track employment outcomes and provide follow-up at 30, 60, 90, and 180 days.
