Job summary

Jade Financial seeks a Restorative Nurse (RN or LPN) to lead the restorative nursing program in our facility. The role serves as the liaison between therapy and nursing, promoting a restorative philosophy across departments to help residents attain their highest function.

The nurse will participate in care planning, supervise restorative measures, and coordinate with therapy to ensure seamless care transitions and adherence to regulatory standards.

Qualifications

• The incumbent must be a nurse registered with the State Board of Nursing.
• The incumbent should possess 3-5 years of equivalent clinical experience.

Responsibilities

• Assists and participates in the development, organization and implementation of the philosophy, objectives, policies, procedures and standards of nursing care.
• Confers with the Director of Nursing/Assistant Director of Nursing in assessing the quality of nursing care and identifying needs for improvement.
• Makes daily rounds to evaluate patient care, monitor nursing practices, and assist Human Resources with nursing and educational needs.
• Performs rehabilitation assessments upon new admissions and for existing patients on a quarterly basis as needed.
• Performs or oversees functional evaluations on admission, quarterly and/or as clinically indicated.
• Consults with nurses in making recommendations for restorative plan of care.
• Works with nurses in developing restorative plan of care.
• Oversees that restorative nursing measures are reflected on the care plan.
• Works closely with the therapy department.
• Coordinates with Human Resources and Director of Nursing to orient new staff to restorative nursing.
