Job summary

Sweden Valley Manor in Coudersport, PA is seeking an RN Restorative Nurse to implement restorative programs and coordinate care using the nursing process. You will document progress with the health care team and ensure adherence to the plan of care across shifts.

Responsibilities include collaborating with licensed therapists, performing functional assessments, educating new staff, and participating in care plan conferences.

Qualifications

• Graduate from an accredited school of nursing.
• Current license as a Registered Nurse.
• CPR certification recommended.

Responsibilities

• Implements restorative nursing programs using the nursing process and ensures proper delivery and documentation with all nursing staff.
• Collaborates with licensed therapists to follow through on recommendations after therapy.
• Performs functional restorative assessments per policy.
• Ensures restorative nursing measures are reflected in the plan of care.
• Participates in care plan conferences and reports on restorative programs.
• Utilizes Electronic Documentation system for restorative nursing documentation and reporting.
• Educates new staff and provides ongoing education on restorative techniques.
• Rounds on all shifts to monitor programs and progress.
• Communicates with the Director of Nursing about progress and issues.
• Participates in weekly QA processes and related meetings.
