Job summary

Elan Skilled Nursing and Rehab in Scranton, PA is seeking a Restorative LPN to coordinate and monitor restorative nursing programs that maintain residents' independence and well-being. You will work with nursing staff, rehabilitation therapists, physicians, and other team members to develop individualized care plans.

The role emphasizes documentation, regulatory compliance, and collaboration with CNAs and therapy teams to promote consistent, high-quality restorative care.

Qualifications

• Graduate of an accredited School of Practical Nursing.
• Current, unrestricted LPN license in the state of employment.
• Current CPR certification required.
• Minimum one year of nursing experience preferred.
• Experience in long-term care, skilled nursing, rehabilitation, restorative nursing, or geriatric care preferred.
• Knowledge of restorative nursing principles, MDS, care planning, and regulatory requirements preferred.

Responsibilities

• Assess residents for restorative nursing needs and identify opportunities to maximize functional abilities.
• Develop, implement, and update individualized restorative nursing programs in collaboration with the interdisciplinary care team.
• Coordinate restorative services including ROM exercises, ambulation and mobility programs, bed mobility and transfer training, and ADL programs.
• Monitor resident progress and modify programs as needed to achieve optimal outcomes.
• Maintain accurate documentation and ensure compliance with federal, state, and local regulations.
• Collaborate with Physical, Occupational, and Speech Therapy departments to promote continuity of care.
• Educate residents, families, and staff regarding restorative nursing goals and interventions.
