Job summary

Elevate Care Des Plaines is hiring a Restorative Nurse (RN or LPN) to develop and implement personalized restorative care plans that promote residents’ independence. You will monitor progress, provide therapeutic activities, and collaborate with the care team in a supportive setting.

Qualified candidates hold an active RN or LPN license and have experience in restorative nursing or long-term care. Strong communication and the ability to work independently are essential.

Your Responsibilities:

Develop & Implement Restorative Care Plans – Work with residents and the healthcare team to promote functional independence.

Monitor & Evaluate Resident Progress – Track the effectiveness of restorative interventions and adjust care plans as needed.

Provide Therapeutic Activities – Assist residents with exercises, mobility, and functional tasks to maintain or improve their abilities.

Educate Residents & Families – Provide guidance on exercises, safety, and strategies to enhance recovery.

Collaborate with Nursing & Therapy Teams – Ensure coordinated care across all disciplines.

Document & Record Resident Progress – Maintain accurate and up-to-date records of care and progress.

Qualifications:

Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) License

Experience in Restorative Nursing, Rehab, or Long-Term Care (Preferred).

Strong Assessment & Critical Thinking Skills.

Excellent Communication & Teamwork Abilities.
