Job summary

Compassus is seeking a Weekend Hospice Admissions Coordinator in Reston, VA to coordinate patient referrals and admissions, ensuring all hospice requirements are met. This on-site weekend role requires availability on Saturdays and Sundays, with 8am–5pm or 8am–8pm shifts.

You will enter admission data into a third-party system, arrange RN visits, inform the IDT and providers of appointment times, and support referral sources.

Qualifications

• High school diploma or GED required; Bachelor's degree preferred.
• 2–3 years in related work strongly preferred; equivalent experience considered.
• Experience communicating with physicians, nurses and healthcare personnel is highly preferred.
• Experience in insurance and Medicare/Medicaid billing and reimbursement is preferred.

Responsibilities

• Support designated program area and/or region.
• Attends and participates in staff meetings, educational programs and community events as requested.
• Receives and processes referrals from physicians, patients, families, friends, and community; contacts physicians to secure admission orders.
• Schedules appointments for informed consents and RN assessment visits; explains hospice services to patients and families.
• Notifies IDT and ancillary providers of appointment times; completes admission per policy; enters data into third-party system.
• Markets referral sources and maintains relationships with long-term care facilities, hospitals and physicians.
