Catholic Charities Eastern Washington is seeking a dedicated Respite support specialist to assist clients in the House of Charity’s Respite program. You will help boost social determinants of health through education and activities, guiding a smooth transition to independent living.

The role requires coordinating intakes, keeping accurate notes, and collaborating with the Behavioral Health and Case Management teams.

Formación

• Bachelor’s degree in social services or related field and one year of relevant direct client services experience; OR AA Degree with 3 years of relevant experience; OR 5 years of relevant experience in direct client services.
• Valid Driver’s License and ability to drive for work use. Passed background check.
• Regularly sit, stand, lift up to 50 pounds, and perform required physical tasks.

Responsabilidades

• Coordinate intakes with hospitals, clinics, and providers; prepare beds for incoming Respite clients.
• Enter daily attendance and complete service notes in required databases.
• Complete intakes, enrollments, discharge plans, and exits per requirements.
• Assist clients with independent living skills, including financial and life-skills coaching.
• Collaborate with Respite Peer Specialist to coordinate activities and community engagement.
• Maintain client files per funder requirements and ensure HMIS data is current.
• Monitor and maintain a clean, safe environment and ensure Respite Rules are observed.
• Respond to emergencies and assist in crisis situations.
