You will build client-centered plans, liaise with landlords, and connect clients to health and social services.

The role emphasizes dignified support, adherence to HIPAA and COA standards, and collaboration with a multidisciplinary team to promote independent living and stability.

Qualifications

• BA in Psychology or Social Work; two years’ experience in mental health preferred.
• Valid Driver’s License and ability to pass background check.
• Confidentiality and HIPAA compliance required.
• Experience coordinating with housing and community resources.

Responsibilities

• Provide supportive housing care coordination for households in CCEW housing or transitioning from hospital settings.
• Visit clients, conduct intakes, develop service plans and coordinate vouchers.
• Coordinate with landlords and treatment teams to address tenancy issues pro-actively.
• Link clients to primary care, substance abuse services, education and employment supports.
