Resources for Human Development (RHD) is seeking a Resource Coordinator / Case Manager for FaSST/Connections CTI in Philadelphia, PA. This full-time role offers a rewarding path to support individuals experiencing homelessness, coordinating services, and promoting recovery and independence.

The position emphasizes advocacy, care coordination, housing resources linkage, and ongoing support, with a team-oriented, trauma-informed approach to help participants overcome barriers and achieve personal

Qualifications

• Bachelor's Degree in a Human Services field.
• Valid Driver's License.
• Strong verbal and written communication skills.
• Computer literacy and ability to use electronic health records, video conferencing, email, and other technology platforms.
• Ability to work independently and as part of a multidisciplinary team.
• Ability to provide trauma-informed, strength-based support.

Responsibilities

• Case management and service coordination; assess needs and connect individuals to services.
• Coordinate mental health, substance use, medical, housing, educational, vocational, and community services.
• Develop and maintain strength-based personal goal plans.
• Monitor service delivery and participant progress.
• Collaborate with treatment providers, shelters, and community organizations.
• Assist participants in accessing benefits and entitlements.
• Advocate for participants within community systems.
• Support participants in achieving personal goals.
• Educate participants regarding available services and resources.
• Promote self-advocacy, recovery, and independence.
• Assist participants in navigating complex systems and overcoming barriers.
