Resources for Human Development is hiring a Case Manager in Rapid City, SD. You will coordinate care, develop and monitor treatment plans, and support youth and families through a collaborative, treatment-focused approach.

The role emphasizes multidisciplinary teamwork, community partnerships, and ongoing professional development. A Bachelor’s degree plus experience is preferred, with DailyPay, competitive benefits, and full-time hours on-site.

Qualifications

• Bachelor’s degree and one year of experience preferred OR equivalent combination of education and experience.
• Intermediate computer skills
• HIPAA compliant home office capability
• Valid driver’s license
• Must pass drug screening and state and federal background checks

Responsibilities

• Develop service plans with the program participant, medical team staff and other stakeholders.
• Maintain relationships with community agencies to connect participants with resources.
• Facilitate development of the Individual Service Plan (ISP) and annual reviews.
• Monitor ISP and document results; intervene as necessary.
• Review and report Critical Incident Reports (CIR) per protocol.
• Participate in Human Rights Committee (HRC) and Behavioral Intervention Committee (BIC) reviews.
• Focus on Personal Outcome measures and Life Course framework knowledge.
• Evaluate caseload needs to support community success.
• Link caseload to programs and supports as needed.
• Document services in Therap with data, reports and plans
