Job summary

City Care Inc. is seeking a Resource Coordinator to assist guests experiencing homelessness with intake, assessments, and connections to wraparound services in Oklahoma City.

You will support clients in reaching goals, coordinating with local services, and maintaining compliance with the Coordinated Entry System. The role involves trauma-informed outreach and data entry across multiple agencies.

Qualifications

• Experience working with adults or children and vulnerable populations.
• Experience navigating poverty, homelessness, mental illness, and recovery.
• Ability to pass background check, driving record review, and drug screening.

Responsibilities

• Carry out the mission, vision, and values of City Care.
• Assist guests with shelter intake, assessments, and documentation collection.
• Complete HMIS data entry and other required documentation.
• Help clients obtain important documents such as IDs, Social Security cards, birth certificates, and proof of income.
• Educate incoming guests on shelter rules, expectations, and policies.
• Develop individualized plans with guests to address unmet needs and overall wellness.
• Connect guests to mental health, physical health, housing, food, and recovery resources.
• Provide case management support, referrals, and resource development.
• Collaborate with Oklahoma City Continuum of Care partners and community agencies.
• Maintain accurate and confidential client files and records.
• Conduct follow-up appointments and maintain consistent engagement with guests.
• Attend weekly staffing and community meetings as designated.
• Uphold professional confidentiality among guests, volunteers, interns, and staff.
