Job summary

City Care Inc. in Oklahoma City seeks a Resource Coordinator to support guests experiencing homelessness with intake, assessments, documentation recovery, and access to wraparound services. Ideal candidates will have experience in homelessness services, strong communication, and the ability to develop individualized plans while maintaining confidential records.

Qualifications

• Experience in case management or advocacy related to homelessness.
• Experience working with adults or children with disabilities.
• Able to pass pre-employment background check, driving record review, and drug screening.
• Must have a current Oklahoma driver’s license and reliable transportation.

Responsibilities

• Carry out the mission, vision, and values of City Care.
• Assist guests with shelter intake, assessments, and documentation collection.
• Complete HMIS data entry and other required documentation.
• Help clients obtain important documents such as IDs and birth certificates.
• Develop individualized plans to address unmet needs and overall wellness.
• Connect guests to mental health, physical health, housing, food, and recovery resources.
• Provide case management support, referrals, and resource development.
• Collaborate with Oklahoma City Continuum of Care partners and community agencies.
• Maintain confidential client files and records.
• Attend weekly staffing and community meetings.
