Aufgaben

• Coordinate Person Centered Planning and ISP development and execution.
• Supervise and train Direct Support Professionals on PCP/ISP implementation.
• Monitor PCP/ISP progress and adjust outcomes based on data and feedback.
• Maintain documentation and monthly outcome reports for Case Manager.
• Ensure services are delivered per PCP/ISP and required standards.
• Conduct on-site evaluations and annual staff evaluations.
• Coordinate benefits (Medicaid, Medicare, SSI, Food Stamps) and community resources.
