Job summary

Reliance Home Health Caregivers is seeking a full-time, in-person supervisor for group homes serving adults with developmental disabilities. Schedule Monday–Friday, 7:00 AM–3:00 PM with a pay rate of $24.00/hour.

Requires a BA in Social Work / Human Services and 2+ years of related experience, including supervisory duties, and CPR/First Aid certification. The role involves coordinating care plans, supervising DSPs, organizing appointments, and ensuring compliance with DHS rules across two 16-bed

Qualifications

Education: BA in Social Work / Human Services or related field.
Two years' experience working with people with developmental disabilities; one year supervisory experience preferred.
Current CPR/First Aid certification is mandatory.
Ability to read, write and understand the English language.
Knowledge of HIPAA guidelines and patient privacy required.
Experience coordinating care and documenting care plans.

Responsibilities

Establish and follow through with a treatment plan for each resident.
De-escalate conflicts between residents and staff.
Plan and coordinate staff training on behavior plans.
Provide and ensure required casework services to each resident.
Coordinate all transportation, medical, therapy, school, volunteer, and parental activities.
Supervise Group Home Workers and integrate therapy programs into the total program.
Maintain agency policies and procedures and Department of Human Services Rules.
