Job summary

UPMC is hiring a Residential Service Coordinator to deliver on-site support for unsheltered behavioral health clients in a Single Room Occupancy setting on the 4th floor of the Second Avenue Commons building. The role requires a Bachelor’s degree in a human services field and a driver’s license, with most time spent in direct client service and active community engagement.

Responsibilities include service coordination, assessments, and crisis-informed collaboration.

Qualifications

• Bachelor's degree in a human services field is required.
• 55% of time should be spent in direct service with clients.
• A safe driving record is required for travel to clients.

Responsibilities

• Advocate for individuals to access necessary services and coordinate care.
• Ensure monthly contact and provide comprehensive service coordination.
• Facilitate service planning meetings and set clear expectations.
• Communicate expectations and ensure ongoing service coordination.
• Assess strengths and needs through interviews and reviews.
• Evaluate services and review cases with relevant parties.
• Develop and maintain relationships with individuals and their networks.
• Serve as primary resource for individuals and families.
• Attend trainings, promote cooperation, and participate in projects.
• Coordinate mental health services as part of a treatment team.
• Promote collaborative planning and maintain community resource catalog.
• Encourage recovery and community inclusion.
• Keep accurate records and provide culturally competent services.
• Utilize at least 55% of time in direct service.
