Job summary

UPMC Service Coordinator I provides mental health service coordination for individuals with serious mental illness and SUD, focusing on assessment, planning, coordination, referrals, and re-evaluation to promote recovery and stability in relationships, housing, and employment.

This grant-funded role offers a $3,000 sign-on bonus with an 18-month commitment and requires a Bachelor's in a Human Services field, a valid driver’s license, and prior community-based experience.

Qualifications

• Bachelor’s degree in a Human Services field required (Social Work, Psychology, or Counseling).
• Other fields considered with ≥12 credits in Human Services.
• One year related experience preferred.
• Community-based experience preferred.
• Safe driving record required.

Responsibilities

• Coordinate substance use services and develop person-centered service plans.
• Assess needs, facilitate service planning meetings, ensure services are delivered effectively.
• Connect individuals and families to community resources and supports.
• Advocate to remove barriers and promote access to needed care.
• Maintain accurate records and ensure regulatory compliance.
• Support continuity of care during hospitalizations and transitions.
• Participate in ongoing training to stay current on best practices and recovery-oriented care.
