Qualifications

• Bachelor’s degree in counseling, psychology, social work, nursing, vocational/psychiatric rehabilitation, education or other mental health field from an accredited college or university.
• Knowledge of mental health and substance abuse systems and supports.
• Ability to work independently.

Responsibilities

• Provide coordination and guidance, counseling, and encouragement to participants transitioning to independence.
• Implement and document participant progress in individualized rehabilitation plans.
• Complete initial and annual assessments and develop recovery plans from a person-centered perspective.
• Assist with case conference presentations and maintain participant case records.
• Liaise with community agencies and families to maintain coordination in the recovery process.
• Assess mental illness symptoms and ensure timely interventions; participate in staff meetings.
