The role focuses on supporting adults with developmental disabilities, behavioral health issues, and substance use recovery by promoting social skills, daily living, and community integration. Responsibilities include documenting progress notes and service plans, coordinating with treatment teams, and assisting with crisis management when needed.

Formación

• High school diploma or GED required.
• 2 years’ experience in the mental health field preferred.
• Strong communication and attention to detail.
• Ability to work independently and as part of a team.

Responsabilidades

• Facilitate skill development in nutrition, self-care, relapse prevention, medication management, money management, and community activities.
• Complete documentation, including progress notes and service plans.
• Establish ongoing dialogue with treatment teams, staff, instructors, medical professionals, and families.
• Perform other duties to enhance program service delivery.
