Job summary

Horizon House, Inc. in Wilmington, Delaware is seeking a Residential Associate Clinician to support participants in transitioning from residential services to greater independence within a community-based setting.

The role includes conducting assessments, developing recovery plans from a person-centered perspective, and maintaining case records. Mileage is reimbursed, orientation occurs at the Philadelphia location, and weekend 16-hour shifts are required.

Qualifications

• Bachelor’s degree in counseling, psychology, social work, nursing, vocational/psychiatric rehabilitation, education or other mental health field from an accredited college or university.
• Knowledge of mental health and substance abuse systems and supports.
• Ability to work independently.
• Good communication skills both oral and written.
• Possess a valid driver’s license with an acceptable driving record.
• Direct experience in mental health services with emphasis on direct care in a community-based setting.

Responsibilities

• Coordinate and guide participants as they transition from residential services to greater independence.
• Implement and document progress in individualized rehabilitation plans.
• Maintain safe, healthy environment to promote growth and development.
• Complete initial and annual participant assessments and develop recovery plans.
• Assist with case conference presentations and manage case records.
• Liaise with community agencies and families to coordinate recovery process.
• Provide ongoing assessment of mental health symptoms and responses to recovery.
• Participate in staff planning, training and meetings.
