Job summary

Horizon House, Inc. in Wilmington, DE is seeking a Residential Support Specialist to join our team at Wilson Road. This part-time, on-site role focuses on coordinating and assisting participants as they move toward independent living within a supportive, community-based setting.

You will conduct person-centered assessments, document progress, engage families and community partners, and participate in planning, crisis intervention, and ongoing staff trainings. Mileage reimbursement is provided.

Qualifications

• Bachelor’s degree in counseling, psychology, social work, nursing, or related mental health field.
• Knowledge of mental health and substance abuse systems.
• Ability to work independently.
• Good oral and written communication skills.
• Valid driver’s license.

Responsibilities

• Coordinate and guide participants as they transition to greater independence.
• Complete initial and annual assessments and develop recovery plans.
• Assist with case conferences for assigned participants.
• Maintain accurate case records for all participants.
• Liaise with community agencies and families to support recovery.
• Assess participants’ mental health symptoms and intervene when needed.
• Participate in staff planning, training, and meetings.
