Job summary

Horizon House, Inc. seeks a Residential Associate Clinician for the New Castle County, Delaware location. This full-time role involves coordinating care, counseling and guiding participants as they transition toward independence within a group home setting.

The position requires a Bachelor’s degree in a mental health field, strong communication skills, and the ability to work independently. Criminal background checks and driving are involved; mileage reimbursement is provided.

Qualifications

• Bachelor's degree in counseling, psychology, social work, nursing, vocational/psychiatric rehabilitation, education or other mental health field from an accredited college or university.
• Knowledge of mental health and substance abuse systems and supports.
• Ability to work independently and communicate well both orally and in writing.

Responsibilities

• Provide coordination, guidance, counseling and encouragement to participants as they transition to more independent living.
• Implement and document progress per individualized rehabilitation plans.
• Maintain safe, healthy environment and accurate case records.
• Liaise with community agencies and families to coordinate recovery processes.
• Assess mental illness symptoms and responses to ensure timely interventions.
• Participate in staff planning, training and meetings.
