Job summary

BHcare seeks a Transitional Counselor for its HARP Residential Program in Connecticut. You will support adults in a 24/7 residential setting, helping them achieve independence and wellness through recovery-focused counseling and skill-building.

Responsibilities include daily living support, goal development, advocacy, and connecting residents to clinical and community resources. Requires two years of direct care experience and a valid driver’s license.

Qualifications

• Two years of college education or a high school diploma/equivalent with two years of direct care experience.
• Valid driver’s license with acceptable driving record and ability to transport clients.
• CPR/First Aid/AED certification required.

Responsibilities

• Provide recovery-oriented support and skill-building for adults in a residential setting.
• Assist residents with daily living activities (personal hygiene, meals, housekeeping, budgeting).
• Develop and implement personalized recovery goals and service plans.
• Advocate, provide emotional support, and counseling using IDDT and MI principles.
• Connect residents with clinical, vocational, educational, and community resources.
