Job summary

BHcare in Connecticut is seeking a part-time Transitional Counselor for the Young Adult Services Residential Program to support residents with recovery and independent living goals.

You will assist with daily living activities, develop personalized recovery plans, build therapeutic relationships, and connect residents to clinical, educational, vocational and community resources, all while maintaining documentation.

Qualifications

• Associate degree or two years of college education in Human Services, Behavioral Science, or related field preferred.
• OR High School Diploma/GED with at least two years of direct care experience in a behavioral health residential setting.

Responsibilities

• Provide individualized support and rehabilitation services to residents.
• Assist with daily living activities including hygiene, meal planning, cooking, cleaning, budgeting, and community integration.
• Develop and implement personalized recovery plans and goals.
• Build therapeutic relationships to foster independence and self-advocacy.
• Utilize recovery-oriented approaches such as Motivational Interviewing and trauma-informed care.
• Provide crisis intervention and connect residents to clinical, educational, vocational, and community resources.
• Maintain accurate documentation and electronic health records.
