Job summary

BHcare is hiring a Transitional Counselor for the HARP Residential Program in Connecticut. The role focuses on recovery-focused counseling, advocacy, and connecting residents to resources.

The schedule is 40 hours weekly with weekend and evening shifts, and the position is onsite as essential staff. Candidates must reside in Connecticut and have two years of direct care experience, plus a valid driver’s license and CPR/First Aid certification.

Qualifications

• Experience supporting individuals with behavioral health, mental health, or substance use challenges.
• Ability to obtain and maintain CPR/First Aid certification.
• Valid driver’s license with acceptable driving record and transportation ability.

Responsibilities

• Assist individuals in developing, implementing, and achieving personalized recovery goals and service plans.
• Provide advocacy, emotional support, and recovery-focused counseling services.
• Connect residents with clinical, vocational, educational, and community resources.
• Facilitate rehabilitation activities and groups to build independent living skills.
• Respond to crises and collaborate with treatment providers, hospitals, and emergency personnel.
• Maintain accurate documentation and health records.
