Coordinates and develops, reviews, and ensures implementation of ISP.

  1. Coordinates and ensures the development of schedules, activities and routines and actively promotes opportunities for individuals to participate in community integrated activities, according to the principles of Person-Centered thinking.
  2. Oversees admissions and referrals.
  3. Works collaboratively with other professionals involved in the individual’s services, such as behavior specialists, communication specialists or therapists, to ensure plans are implemented and progress documented.
  4. Coordinates and completes initial and on-going assessments of individuals.
  5. Develops and ensures implementation of planned program activities.
  6. Coordinates and serves as team leader in multidisciplinary team meetings pertaining to the individual’s service plan.
  7. Collaborates and cooperates with individuals, family members, other provider agencies, and advocates.
  8. Utilizes local resources to promote involvement in community activities, including volunteer opportunities.
  9. Develops educational and training materials, presents educational materials on various aspects of programs and provides assistance to staff related to Person-Centered Planning, ISPs, Outcomes and Self-Determination.
  10. Provides training to new hires on the program planning process, as well as instruction in outcomes documentation, and teaching strategies on outcomes.
  11. Serves as an auditor for employee’s personnel records and training records.
  12. Participates in interdisciplinary team meetings.
  13. Serves as a backup during emergencies.
  14. Monitors compliance to all regulatory standards related to ISPs and programming.
  15. Reports and enters incidents as outlined in the Incident Management Bulletin and LNL’s policy on Incident Management.
  16. Assists in the development of data collection methods to report and monitor program activities and analyzes data to determine if the activities provided are meeting program outcomes.
  17. Completes all assigned administrative tasks relative to the program.
  18. Participates in the company’s on-call system.
  19. Supervises direct support staff.
  20. Provides overall on-site supervision and implementation of the daily operations of the home.
  21. Recommends and takes disciplinary action to ensure quality work performance of immediate staff.
  22. Ensures the individuals’ health, safety and welfare.
  23. Responds to individual’s complaints.
  24. Ensures individual health and medical needs are met.
  25. Monitors and evaluates performance according to the company’s performance management policies and procedures.
  26. Monitors program and staff compliance to all company policies and procedures and all regulatory standards.
  27. Communicates relevant information to families, outside agencies, and other professionals as necessary.
  28. Oversees and maintains records pertaining to inventory, both for company owned property and possessions belonging to individuals, as applicable to the program.
  29. Accounts for all individual funds and petty cash funds; maintains an adequate supply of funds for the individuals and the home, as applicable to the program.
  30. Monitors all non-reportable and reportable incident reports.
  31. Ensures all reportable incidents are finalized in EIM and corrective actions are followed through.
  32. Investigates and follows up on all incidents.
  33. Participates in the corporate decision-making process.
  34. Manages program and staff in compliance with company policies and procedures and all regulatory standards.
  35. Oversees incident management and follow-up.
  36. Oversees the investigation and resolution of program issues and grievances.
  37. Approves leaves, schedules personnel, and authorizes or assigns overtime.
  38. Reviews and evaluates the educational and training needs of staff and arranges for further training.
  39. Audits training and personnel records to ensure all staff are following regulatory requirements.
  40. Consults with program psychiatrist, or other delegated professionals, regarding clinical, medical and programmatic issues.
  41. Ensures appropriate coordination of services/treatment and follow-up.
  42. Oversees daily programs and/or staff schedules.
  43. Oversees the development of data collection methods to report and monitor program activities, analyzes data to determine if the activities provided are meeting program goals.
  44. Operationalizes compliance with sanctioned philosophical and conceptual approaches.
  45. Participates in quality assurance/quality management (QM) activities related to the programs specified in the QM plan.
  46. Develops program policy and helps to coordinate it with overall company and program goals, directs the daily operations, evaluates results, and recommends changes in programs.
  47. Responds to emergency situations and provide leadership.
