Job summary

InterCommunity, Inc. is a Federally Qualified Health Center Look-Alike (FQHC LA) serving Hartford area, offering behavioral health services and primary care access.

This role provides milieu supervision, care coordination, and transportation support within a residential program, with a schedule Sunday-Thursday 3:00 pm–11:30 pm. Ideal candidates hold a Peer Support Specialist Certification or related degree, have 3 years of residential experience, and possess CPR certification and a valid driver

Qualifications

• Peer Support Specialist Certification, or Associate's or Bachelor's Degree in Human Services Field.
• 3 years of experience in residential setting preferred.
• Working knowledge of substance abuse and/or mental health treatment.
• CPR Certification within 60 days of start date.
• Valid Driver License.

Responsibilities

• Provide milieu supervision of the program and participants, coordinating and facilitating social-rehabilitation groups and programs.
• Manage clients and ensure facility safety and security, including substance abuse screening as directed.
• Conduct individual and group social-rehabilitation and care coordination activities with a caseload of identified clients.
• Supervise participants in milieu and during on-site and scheduled off-site activities; perform room safety checks.
• Maintain confidential progress notes and other documentation.
• Provide necessary transportation and program-related errands (agency or personal vehicle).
• May work past scheduled hours if staff shortage occurs until relief staff is available.
• Perform other related tasks as required.
