Job summary

RADIAS Health is seeking a Mental Practitioner Supervisor to oversee a four-resident group home within the Residential Support Services program in Minnesota. The role includes supervision of staff, coordination of care, and development of treatment plans in collaboration with the treatment team.

Responsibilities include managing client schedules, coordinating medical and behavioral plans, and ensuring compliance with DHS and county regulations.

Qualifications

• Must have car, auto insurance and valid driver’s license.
• Experience coordinating client care, treatment plans, and licensing/audit processes.

Responsibilities

• Coordinate client care, including quarterly case conferences with the case manager and treatment team.
• Develop and review quarterly Individual Service Plans and related plans with program leadership.
• Coordinate medical appointments and transportation for clients.
• Assist with admission/discharge processes and community referrals.
• Lead weekly house staff meetings and maintain incident reports.
