Job summary

Radias Health is seeking a Mental Practitioner Supervisor to lead the RSS program and supervise staff in a group home setting. You will coordinate client care, update plans, and liaise with clinicians, hospital staff and community partners to support recovery and community integration for adults with serious mental illness and co-occurring substance use disorders.

Qualifications

• Coordinate client care with case manager, treatment team and family when needed.
• Develop, implement and review quarterly treatment and abuse prevention plans.
• Coordinate medical appointments, referrals and transportation for clients.

Responsibilities

• Coordinate client care with the case manager and treatment team.
