Job summary

PathWays of Southwestern Pennsylvania seeks a Residential Home Coordinator to lead a home, ensuring high-quality, person-centered care. You’ll connect daily operations with the Residential Program Manager, manage staff coverage, and maintain compliance.

You will on-call and contribute 20–24 hours of direct care weekly, assisting with daily living skills, groceries, meal planning, and documentation while fostering a supportive, safe home environment.

Qualifications

  • High School Diploma or GED – required.
  • At least 1 year of direct care experience – required.
  • Valid driver’s license and clean Motor Vehicle Record.
  • Ability to complete background checks (we’ll help!).
  • Pre-employment physical and TB test.

Responsibilities

  • Overseeing daily home operations and ensuring proper staff coverage.
  • Completing and monitoring required documentation (medication logs, IDT notes, data sheets, care plans, incident reports, EIM records, petty cash, chore charts).
  • Reviewing staff documentation for accuracy and compliance.
  • Managing grocery purchasing, food storage, and meal planning within budget.
  • Bi-weekly household inspections for cleanliness and safety.
  • Communicating with Residential Management about needs, health concerns, and staff performance.
  • Collaborating on menus and nutritional needs with Nursing Services.
  • Serving on the Interdisciplinary Team to support individualized plans.
  • Direct care 20–24 hours per week as needed.
  • Providing hands-on support with daily living skills and community activities.
  • Training and mentoring staff to ensure quality care.

What You’ll Be Doing as a Residential Home Coordinator

As a Residential Home Coordinator, you are the heart of your assigned home — keeping operations running smoothly while ensuring high-quality, person-centered care. You’ll serve as the link between daily home operations and the Residential Program Manager, ensuring staff coverage, compliance, and the overall well-being of the individuals supported.

On-Call Expectations
  • On call 24 hours a day, Monday–Friday for your assigned home
  • Participation in a rotating weekend on-call schedule
A Few Things You’ll Be Responsible For
  • Overseeing daily home operations and ensuring proper staff coverage
  • Completing and monitoring required documentation, including:
    • Medication logs, IDT notes, data sheets, healthcare plans
    • Incident/observation reports, EIM reports, petty cash records, chore charts
  • Reviewing staff documentation for accuracy, completeness, and compliance
  • Managing grocery purchasing, food storage, and meal planning within budget
  • Conducting bi-weekly household inspections for cleanliness and safety
  • Maintaining open communication with Residential Management regarding:
    • Individual needs
    • Health and safety concerns
    • Compliance and staff performance
  • Collaborating with the Adult Services Director of Nursing Services on:
    • Menus, calorie counts, dietary restrictions, and nutritional needs
  • Serving as a member of the Interdisciplinary Team to support individualized plans
  • Working 20–24 hours per week in direct care (floor time) as needed
  • Providing hands-on support with daily living skills, including:
    • Personal care, finances, shopping, meals, housekeeping, and laundry
  • Supporting meaningful recreation, leisure activities, and community outings
  • Training and mentoring new and current staff to ensure quality, consistent care
