Job summary

Horizon House, Inc. in Hatboro, PA, seeks an On-Call Crisis Worker in its Behavioral Health Services Division to support residents experiencing mental health crises who are not a immediate risk.

The role provides guidance to help individuals learn to live as independently as possible in the community and to participate in developing and implementing individualized program plans. Responsibilities include admitting participants, documenting, orienting to routines, monitoring health changes,

Qualifications

• High school diploma or equivalent with 3 years of mental health direct care experience or equivalent.
• Knowledge of mental health systems and supports.
• Knowledge of privacy regulations and guidelines about access, use, and disclosure.
• Good oral and written communication skills.
• Must have a valid driver’s license and acceptable driving record.

Responsibilities

• Accepts assigned participant(s) at admission, completes documentation, and orients participant to facility routines.
• Become familiar with relevant personal, familial, social, vocational, and cultural data and observe significant changes in participants’ mental and physical health.
• Monitor and support participants focusing on personal and treatment goals; counsel participants to help them address problems, values, responsibilities, and relationships; assist with personal futures planning.
