Job summary

Open Sky Community Services seeks a Residential Counselor to provide person-centered support in residential and community settings in Worcester, MA. You will assist individuals with daily living activities, transportation, and community participation while promoting independence and meaningful community involvement.

Key responsibilities include developing individualized service plans, maintaining a safe environment, and documenting progress.

Qualifications

• High School Diploma, GED or equivalent is required.
• Valid Driver’s License and acceptable driving record required.
• Reliability, willingness to learn, and openness to new opportunities.

Responsibilities

• Assist in developing individualized, person-centered service plans.
• Provide support with daily living activities, skill development, and personal goal achievement.
• Assist individuals with accessing transportation and community resources.
• Implement activities that promote community inclusion, relationship building, and personal growth.
• Maintain a safe, clean, and supportive environment; ensure health and wellbeing of individuals served.
• Review, complete, and maintain required documentation accurately and timely.
