Job summary

Open Sky Community Services is seeking a Residential Counselor in Massachusetts to support individuals in achieving greater independence and quality of life in residential and community settings. You will provide direct care, guidance, and skill-building support while promoting safety and person-centered planning.

Responsibilities include developing individual plans, assisting with daily living activities, transportation access, and community inclusion.

Qualifications

• High School Diploma, GED or equivalent is required.
• Valid Driver’s License and acceptable driving record are required.
• Reliability, willingness to learn, and openness to new opportunities.

Responsibilities

• Assist in developing individualized, person-centered service and support plans and play an active role in their implementation.
• Provide support with daily living activities, skill development, and personal goal achievement.
• Assist individuals with accessing transportation and community resources as needed.
• Implement activities that promote community inclusion, relationship building, personal growth, and the development of valued social roles.
• Maintain a safe, clean, and supportive environment that promotes health, safety, and well-being of individuals served.
• Review, complete, and maintain all required documentation in an accurate and timely manner.
