Qualifications

• Valid Driver’s License and acceptable driving record are required.

Responsibilities

• Assist in developing individualized, person-centered service plans and support their implementation.
• Provide support with daily living activities, skill development, and personal goal achievement.
• Assist individuals with transportation and community resources as needed.
• Implement activities that promote community inclusion, relationship building, personal growth, and social roles.
• Maintain a safe, clean, and supportive environment for the individuals served.
• Review, complete, and maintain required documentation in a timely manner.
